DOPS OD is a prescription medicine from Sumitomo Pharma (Osaka, Japan) containing droxidopa, a synthetic amino acid precursor of noradrenaline. What droxidopa is used for

Approved indications in Japan:

Parkinson's disease, Yahr severity stage 3 — improvement of frozen gait (すくみ足) and dizziness on standing

Shy-Drager syndrome and familial amyloid polyneuropathy — improvement of orthostatic hypotension, syncope and dizziness on standing

Patients on haemodialysis with orthostatic hypotension — improvement of dizziness, light-headedness, unsteadiness, fatigue and weakness

Droxidopa is converted in the body to noradrenaline and raises blood pressure on standing; it does not treat the underlying neurological disease itself.

How it is used

DOPS OD is a prescription medicine: the dose, titration and duration are set by the treating physician, with blood-pressure monitoring. The Japanese prescribing information describes the following regimens for supervised use:

Parkinson's disease: start at 100 mg once daily by mouth, increase by 100 mg every other day, standard maintenance dose 600 mg per day in three divided doses; up to 900 mg per day.

Orthostatic hypotension, syncope, dizziness on standing: start at 200–300 mg per day in two or three divided doses, increase by 100 mg per day every few days to one week, standard maintenance dose 300–600 mg per day in three divided doses; up to 900 mg per day.

Haemodialysis patients: 200–400 mg taken 30–60 minutes before dialysis, maximum 400 mg per dose; treatment is stopped if no effect is seen within one month.

Doses are not taken close to bedtime, and long-term treatment is not stopped abruptly.

Possible side effects

Reported effects include nausea, headache, dizziness, palpitations, dry mouth, insomnia, diarrhoea and raised blood pressure while lying down (supine hypertension). Reactions described as serious in the Japanese label include neuroleptic malignant syndrome (high fever, impaired consciousness, severe muscle rigidity, involuntary movements, raised serum CK) and blood disorders (leukopenia, agranulocytosis, neutropenia, thrombocytopenia). Seek medical help if these occur.

Important precautions

Not to be combined with halogen-containing inhaled anaesthetics (such as halothane) or with catecholamine preparations.

Contraindicated in angle-closure glaucoma, in pregnancy or possible pregnancy, and in severe peripheral vascular disease in dialysis patients.

Blood pressure is measured lying and standing during treatment; supine hypertension needs medical attention.

Tell the physician about any MAO inhibitor, sympathomimetic or blood-pressure medicine taken at the same time.

Storage

Store at room temperature, away from direct light and moisture, in the original pack and out of reach of children. Check the expiry date printed on the box.
