🎯 Indication

Pirfenex 200mg is indicated for the treatment of idiopathic pulmonary fibrosis (IPF) in adults.

  • It does not cure the disease but helps slow the decline in lung function and reduces the rate of scar tissue formation.
  • The drug has anti-inflammatory and anti-fibrotic properties.

💊 Dosage and Administration

The medication must be taken strictly as prescribed by a physician. The standard regimen involves a gradual dose escalation to improve tolerability.

  • Initial dose: 200 mg (1 tablet) three times a day.
  • Dose titration: The dose is increased by 200 mg per intake each week until the maintenance dose is reached.
  • Maintenance dose: 600 mg (3 tablets) three times a day (total daily dose of 1800 mg).
  • Important: Take with or after food, as taking it on an empty stomach increases blood concentration and the risk of side effects.

⚠️ Common Side Effects

The most common side effects include:

  • Gastrointestinal: Nausea, vomiting, diarrhea, loss of appetite, weight loss.
  • Skin: Rash, severe photosensitivity (reaction to sunlight and UV exposure).
  • Nervous system: Headache, dizziness, drowsiness, insomnia.
  • Liver: Elevated liver enzymes (ALT, AST). Regular monitoring of liver function is required.

🚫 Contraindications

This medication is contraindicated in the following cases:

  • Allergy to pirfenidone or any component of the formulation.
  • Severe liver or kidney impairment (including patients on dialysis).
  • Pregnancy and breastfeeding.
  • Concomitant use of fluvoxamine (an antidepressant).

  1. Photosensitivity: Avoid prolonged sun exposure during treatment. Use high-protection sunscreen and wear protective clothing to reduce the risk of severe sunburn.
  2. Liver Function Monitoring: Regular blood tests are essential to monitor liver enzymes throughout the treatment.
  3. Driving and Machinery: The medication can cause dizziness and significant drowsiness. Caution is advised when driving or operating machinery.
  4. Smoking: Smoking reduces the effectiveness of pirfenidone. It is recommended to quit smoking during treatment.
  5. Drug Interactions: Inform your doctor about all medications you are taking, especially antibiotics (ciprofloxacin) and medications for heartburn (omeprazole), as they may affect how Pirfenex works.
