Thymalin: A Peptide Bioregulator for the Reactivation of Cellular Immunity

The immune system is an army that daily defends the body against viruses, bacteria, fungi, and its own malignantly transformed cells. The central command organ of this army is the thymus gland, where T-lymphocytes undergo maturation and "training." With age, following severe infections, stress, radiation, and chemotherapy, thymic function becomes suppressed, and immune surveillance weakens. Thymalin is an original peptide bioregulator composed of a complex of low-molecular-weight polypeptides extracted from calf thymus. It possesses the ability to precisely restore the impaired links of cellular immunity, returning to the body the capacity to effectively resist disease.

🔬 Biochemical Identity and Mechanism of Action

Thymalin is not a foreign stimulant that "whips" an exhausted immune system into action. It operates as a physiological regulator, replenishing the deficit of the body's own thymic peptides — substances normally produced by thymic epithelial cells that orchestrate the maturation and differentiation of T-lymphocytes.

The mechanism of action of Thymalin unfolds on several levels:

1. Restoration of the T-Cell Compartment.

Thymalin stimulates the proliferation and differentiation of T-lymphocyte precursors into mature functional subpopulations: T-helper cells (CD4+), cytotoxic T-lymphocytes (CD8+), and T-regulatory cells. This results in the normalization of the immunoregulatory index (CD4/CD8 ratio) — a key parameter of cellular immunity status.

2. Activation of Phagocytosis.

The drug enhances the engulfment and bactericidal activity of macrophages and neutrophils — the first-line defense cells responsible for eliminating pathogens.

3. Stimulation of Interferon Production.

Thymalin upregulates the synthesis of endogenous alpha- and gamma-interferons, reinforcing antiviral and antitumor defenses.

4. Acceleration of Regeneration.

The drug stimulates tissue repair and healing processes, including the recovery of bone marrow following chemo- and radiotherapy, thereby promoting a faster restoration of hematopoiesis.

📋 Clinical Indications

Thymalin is prescribed across a broad spectrum of pathologies united by a common thread — immune dysfunction:

1. Immunodeficiency States:

  • Primary and secondary immunodeficiencies with predominant T-cell involvement.
  • Age-related decline in immunity (immunosenescence).

2. Infectious Pathology:

  • Protracted and chronic viral infections (herpes, cytomegalovirus, Epstein-Barr virus).
  • Frequently recurrent bacterial infections of the ENT organs and respiratory tract.
  • Pulmonary tuberculosis (as part of combination therapy).
  • Chronic viral hepatitis B and C.

3. Oncological Practice:

  • Immunorehabilitation following courses of chemo- and radiotherapy.
  • Restoration of blood counts and immune status in cancer patients.

4. Purulent-Septic Processes:

  • Peritonitis, sepsis, osteomyelitis, phlegmon, abscesses.
  • Extensive burns and frostbite.

5. Rehabilitation:

  • Following severe trauma and surgical interventions.
  • Acceleration of the healing of trophic ulcers and long-standing non-healing wounds.

6. Dermatology:

  • Chronic recurrent pyoderma, furunculosis.
  • Psoriasis (in combination therapy).

💉 Administration and Dosing Regimen

Thymalin is supplied as a lyophilized powder (lyophilisate) for the preparation of an intramuscular injection solution.

  • Solution Preparation: The contents of the vial are dissolved in 1–2 ml of isotonic sodium chloride solution (0.9% NaCl) or water for injection immediately prior to administration.
  • Route of Administration: Intramuscular.
  • Single Dose for Adults: 5–20 mg.
  • Daily Dose: 5–20 mg, administered once.
  • Frequency: Daily.
  • Course Duration: From 3 to 10 days, depending on the severity of the condition. If necessary, courses are repeated at intervals of 1–6 months.

Treatment Protocols:

  • For acute infections and purulent-septic processes: daily for 5–7 days.
  • For chronic diseases: daily for 7–10 days, followed by a transition to a maintenance regimen (1 injection every 3–4 days).
  • In oncological practice: courses before and after chemotherapy for the prevention of myelosuppression.

⚠️ Contraindications

Thymalin is not prescribed in the setting of:

  • Individual hypersensitivity to the drug's components.
  • Atopic bronchial asthma (due to the potential risk of allergic reactions).
  • Pregnancy and lactation (clinical safety studies have not been conducted).
  • Autoimmune diseases in the acute exacerbation phase without immunological monitoring — the decision is made by the physician on an individual basis.

📊 Tolerability Profile

Thymalin is generally well tolerated. In rare instances, the following may occur:

  • Allergic reactions (skin rash, pruritus).
  • Soreness, redness, and infiltration at the injection site.
  • A short-term rise in body temperature to subfebrile levels (interpreted as an immune activation response).

📦 Storage Conditions

The lyophilisate must be stored in a dry, light-protected location at a temperature not exceeding 25 °C (77 °F). The prepared solution must be used immediately and cannot be stored. Keep out of the reach of children.
