Cerebrolysin 2 ml: A Peptide Neuroprotectant for Comprehensive Cerebral Rehabilitation

The brain is an organ whose regenerative potential was long considered exhausted after the completion of embryonic development. However, the discovery of neurotrophic factors and the phenomenon of adult neurogenesis overturned this dogma, catalyzing the creation of fundamentally novel neuroprotective strategies. Cerebrolysin is an original Austrian pharmaceutical preparation consisting of a purified hydrolysate of porcine brain proteins, containing a complex of low-molecular-weight biologically active neuropeptides and amino acids. It is supplied as a sterile injectable solution (2 ml per ampoule, 10 ampoules per pack). The drug acts multimodally, mimicking the endogenous neurotrophic support that the brain receives during its developmental period and triggering cascades of neurorepair and neuroplasticity.

🧠 Molecular Mechanism: Neurotrophic Mimicry

Cerebrolysin is neither a neurotransmitter, nor a receptor agonist/antagonist, nor a classical nootropic. Its uniqueness lies in its ability to mimic the signals of endogenous neurotrophic factors (NTFs) and to simultaneously target multiple pathogenetic cascades of neuronal injury:

1. Activation of Neurogenesis and Neuroprotection.

Cerebrolysin peptides are structurally and functionally related to BDNF (brain-derived neurotrophic factor), GDNF (glial cell line-derived neurotrophic factor), NGF (nerve growth factor), and CNTF (ciliary neurotrophic factor). By binding to cognate receptors (Trk receptors), they trigger intracellular signaling cascades (PI3K/Akt, MAPK/ERK) that block apoptosis, stimulate the differentiation of neural stem cells, and enhance axonal and dendritic outgrowth.

2. Suppression of Excitotoxicity.

Cerebrolysin reduces the pathological release of glutamate into the synaptic cleft and diminishes the sensitivity of NMDA receptors to it, preventing the glutamate-calcium overload of neurons — the key mechanism of cell death during ischemia and trauma.

3. Blockade of Apoptotic Cascades.

The drug inhibits the activation of caspases (caspase-3, caspase-9) and downregulates the expression of the pro-apoptotic protein Bax, while simultaneously upregulating the anti-apoptotic Bcl-2. This prevents both acute and delayed neuronal death within the ischemic penumbra.

4. Improvement of Microcirculation and Metabolism.

Cerebrolysin enhances the efficiency of glucose and oxygen utilization by neurons, stimulates angiogenesis in ischemic regions, and improves the rheological properties of blood.

📋 Clinical Indications

Cerebrolysin is deployed in neurological, psychiatric, neurosurgical, and pediatric practice:

1. Acute and Chronic Cerebrovascular Disorders:

  • Ischemic stroke (acute phase and recovery period).
  • Chronic cerebral ischemia, dyscirculatory encephalopathy.
  • Vascular dementia.

2. Neurodegenerative Diseases:

  • Mild to moderate Alzheimer's disease.
  • Mixed vascular-neurodegenerative dementias.

3. Sequelae of Traumatic Brain Injury:

  • Cognitive deficits of post-traumatic origin.
  • Post-concussion syndrome.

4. Psychiatric Pathology:

  • Treatment-resistant depression (as adjuvant therapy).
  • Cognitive deficits in schizophrenia.

5. Pediatric Neurology:

  • Delayed psychomotor and speech development.
  • Cerebral palsy.
  • Attention deficit hyperactivity disorder (ADHD).

💉 Administration and Dosing Regimen

Cerebrolysin is a clear, amber-colored solution.

  • Route of Administration: Intramuscular or intravenous (slow bolus or drip infusion in 100–250 ml of normal saline). IV infusions are performed in a hospital setting.
  • Dosage and Course Duration depend on the pathology and are determined by the physician:
    • Acute stroke, TBI: 20–50 ml/day IV drip for 10–20 days.
    • Chronic cerebral ischemia, dementia: 5–10 ml/day IM or 10–20 ml/day IV for 20–30 days.
    • Pediatrics (developmental delay): 0.1–0.2 ml/kg body weight IM for 20–30 days.
  • Repeat Courses: 2–3 times per year.

⚠️ Contraindications and Precautionary Measures

Cerebrolysin is not prescribed in the setting of:

  • Status epilepticus and epilepsy with frequent generalized seizures.
  • Severe renal failure.
  • Individual hypersensitivity to the drug's components.
  • Pregnancy and breastfeeding.

Special Instructions:

  • The drug should be used with caution in patients with allergic diathesis and bronchial asthma.
  • Overly rapid IV administration may provoke a sensation of heat, sweating, and dizziness.
  • Cerebrolysin must not be mixed in the same infusion container with solutions that alter pH (outside the 5.0–8.0 range) or with lipid-containing solutions.

📊 Tolerability Profile

Cerebrolysin demonstrates good tolerability. In rare instances:

  • A sensation of heat, sweating, and mild dizziness upon rapid IV administration.
  • Allergic reactions (skin rash, pruritus).
  • Dyspeptic symptoms (nausea).
  • Agitation, sleep disturbance (if administered late in the day).

📦 Storage Conditions

Ampoules must be stored in a light-protected location at room temperature (not exceeding 25 °C / 77 °F). Do not freeze. Keep out of the reach of children.
