General Information

Penicillamine is a chelating agent and immunomodulator used to remove excess metals such as copper from the body and to manage autoimmune conditions. It is particularly effective in the treatment of Wilson’s disease, rheumatoid arthritis, cystinuria, and certain heavy metal poisonings.

Indications

• Wilson’s disease — promotes copper excretion
• Rheumatoid arthritis — for patients intolerant to or unresponsive to first-line therapy
• Cystinuria — reduces cystine stone formation
• Heavy metal poisoning (lead, mercury) — as part of chelation therapy

Pharmacological Action

• Chelates copper and heavy metals, forming soluble complexes
• Reduces inflammation and modulates immune response
• Lowers cystine concentration in urine, preventing kidney stones

Dosage and Administration

Dosage is individualized depending on the condition:

• Wilson’s disease: 0.75–1.5 g/day
• Rheumatoid arthritis: gradual dose escalation
• Cystinuria: 1–2 g/day
• Heavy metal toxicity: customized regimen

Capsules should be taken on an empty stomach — 1 hour before meals or 2 hours after.

Contraindications

• Hypersensitivity to penicillamine
• Pregnancy and breastfeeding
• Bone marrow suppression
• Severe renal impairment
• Systemic lupus erythematosus
